Student’s guide for the Neptun request

Dear Student,
We would like to let you know that the deadline of uploading your request for certifying the vaccination against coronavirus or the exemption from vaccination has been extended to 24:00 19th September, 2021 (Sunday midnight).
In case your request has already been accepted please do not submit it again!
How to check the information: Neptun – Administration – Request – Available request forms tab – press + button on right beside the request form.
If only „Correction” link works, you can read a message in a pop-up window how to upload your document again.
If „View resolution” link is available, you can find the final printable answer for your request there.
Those who need to make a correction or upload their documents for the first time, please upload all the necessary documents (a certification with your name and vaccination data or the medical opinion of the Center for Occupational Medicine). Without adequate documents your request may not be accepted.
Furthermore we would like to call your attention to the fact that those students who fail to submit the request or do not make the correction of their request (status of request remains „Sent back for correction”) by the extended deadline, may not certify their entitlement of participating the education towards the practice leader.
